Physicians Plus wants to help our members with heart failure (HF) live healthier, more active lives. Members who have HF and wish to reduce their risk of health problems may be eligible for our HF nurse case management program at no cost. This innovative program uses an in-home Cardiocom® Telescale® for daily weight and symptom monitoring. The HF case management program enables early detection of serious health concerns and can help prevent future hospitalizations.

Who is Eligible?
Physicians Plus members, age 18 and older:
  • With heart failure and symptoms such as shortness of breath and/or leg swelling and a willingness to improve their health; or
  • As suggested by their health care provider.
How Does it Work?
  • A Meriter Home Health nurse will help monitor the member's heart function and identify health problems.
  • Every morning, the member will use the Telescale in their home to weigh themselves and answer questions about their current symptoms.
  • Their information is recorded and sent to Meriter Home Health using a standard telephone line.
  • A Meriter Home Health nurse may call the member to verify their answers and weight and to gather additional information.
  • If there is a weight change or a symptom that needs follow-up, the Meriter Home Health nurse will notify the member's doctor.
Questions "Asked" by the Telescale:
  • Are you feeling more short of breath?
  • Are you coughing more than usual?
  • Are your ankles or feet more swollen?
  • Does your stomach feel more bloated?
  • Are you urinating less than usual?
  • Are you reducing your sodium (salt)?
  • Are you having more chest discomfort?
  • Are you more dizzy or lightheaded?
  • Are you more tired than usual?
  • Did you exercise yesterday?
  • Are you taking your medication?
