If this is the second, third or fourth year you are completing the on-line Succeed health risk assessment, it will appear under the Coaching Sessions tab on your GO-TO Healthy Choices home page. Past reports will be available to you under Understand My Health Risks, HealthMedia Succeed. Click on “GO” to get started with your Succeed Health Risk Assessment.
