New User Registration Instructions ** Print this instruction page for reference, then proceed to registration.
- Have your Physicians Plus member ID card handy. Please note that an email address is required for GO-TO registration. Free email addresses are available from a number of Internet sites such as www.Yahoo.com and www.Hotmail.com.
- Read the terms and conditions on the current page and click I Agree to continue.
- Enter the information requested on the form.
- Enter all six numbers of your Group Number (GROUP#), located in the upper left hand corner of your membership card).
- Enter your Member ID Number (MEMBER#), located on the left side of your membership card and the two-digit "person code" (listed to the left of your name on your membership card).
- Enter your last name.
- Enter your date of birth, using four digits for the year (2009, not 09) and click Find My Record. NOTE: If you get an error message at this point, please make sure you entered all above information accurately.
- Next, create a user name and password using any combination of at least six letters or numbers you can remember easily. You should also set up a "hint" question and answer that can prompt you to remember a forgotten password. Click Submit and go to the next screen.
- At the welcome page, click Return to Home Page, enter your new user name and password and click Log In. Click I Agree at the bottom of the user agreement to be directed to your personal home page. Your GO-TO registration is complete.
