The Eat Healthy Rebate allows members to apply their Good Health Bonus reimbursement to the cost of a produce share from a Madison Area Community Supported Agriculture Coalition (MACSAC), FRESH Food Connection, or Simply Wisconsin farm.
To be eligible for the Eat Healthy Rebate, you must order a produce share (meat, eggs and dairy purchases are not eligible for reimbursement) from a MACSAC, FRESH Food Connection, or Simply Wisconsin farm. Single contracts are eligible for a $100 rebate; family contracts are eligible for a $200 rebate. You must be a Physicians Plus member at the time your produce share is purchased.
To receive your Eat Healthy Rebate:
- Write “P+ Eat Healthy Rebate,” along with your date of birth and phone number on your MACSAC, FRESH Food Connection, or Simply Wisconsin farm sign-up form.
- Send your farm sign-up form and payment to the farm according to the instructions on the form.
- Send a copy of your form and proof of payment to:
