Good Health Bonus
A $100 or $200 Good Health Bonus reimbursement. How will you earn yours?
Members with single contracts can receive up to $100 per year. Members with family contracts can receive up to $200 per year per family. Please allow 6-8 weeks to receive your Good Health Bonus reimbursement reward. The Good Health Bonus program is not available to BadgerCare Plus members.
For most programs, you will need to download the Reimbursement Request Form and mail it to the address listed on the form. Please note: Good Health Bonus submissions for the 2011 calendar year must have been postmarked by 1/31/2012.

Take the Succeed Health Assessment & Complete Digital Coaching Sessions
Take the Succeed Health Assessment and earn $25. Then complete two digital coaching sessions and their 30-, 90-, and 180-day post-program surveys and earn an additional $75. GO-TO Healthy Choices rewards are deducted from your annual Good Health Bonus maximum. You must have a GO-TO account to take the Succeed Health Assessment and complete the digital coaching sessions.
Get a Gym Reimbursement. Join a Health & Fitness Facility.
To receive a gym reimbursement for your Health and Fitness Facility membership, simply collect receipts for enrollment or monthly payments until they total at least your maximum allowance. Attach a copy/copies of your receipt(s), complete the Reimbursement Request Form and mail it to the address listed on the form.
To be eligible for class reimbursement:
- Participants must attend the entire class or 75% of classes offered in a series.
- Courses must be provided by facilities approved by Physicians Plus. Please note that not all courses that a facility offers are approved courses. Courses may be cancelled or rescheduled if minimum enrollment is not achieved.
Class participants must first pay the course registration fee. Print the Reimbursement Request Form and bring it with you to class. You will need your instructor’s signature on the form before mailing it to the address listed on the form. If you prefer, you may also get this form from your instructor the day of the class.
With the Eat Healthy Rebate, members can apply their Good Health Bonus rebate to the cost of a CSA produce share from a FairShare CSA Coalition, FRESH Food Connection, Simply Wisconsin farm, or the Farley Center Farm Incubator.
